The total loss of engine power resulting from the fatigue failure of the engine's number 2 cylinder exhaust valve. The fatigue failure was due to valve guide wear that led to excessive clearance between the valve and valve guide. Contributing to the accident was the contract operator’s lack of compliance with its own maintenance procedures, which, if followed, would have prevented the accident.

Oct 21, 2009 Provo, UT N6869R Fatal (1) substantial A total loss of engine power due to fuel starvation as a result of the pilot’s inadequate preflight inspection.
Sep 24, 2009 Hilltop Lakes, TX N9734Y Fatal (1) destroyed The loss of control of the airplane in dark night light conditions due to the pilot’s spatial disorientation.
Apr 09, 2009 Edinburg, TX N127TC Fatal (1) destroyed A loss of engine power for undetermined reasons. Contributing to the accident was the pilot's failure to select a suitable alternate landing site.
