The pilot's loss of control and failure to attain/maintain adequate airspeed during an attempted go-around after a hard bounced landing, resulting in an aerodynamic stall. Contributing to the accident was the pilot’s recent minimal experience in airplane make/model and in operations at the grass airstrip.

Mar 18, 2010 Lewisville, TX N56489 Fatal (1) substantial The pilot did not retract the landing gear before a water landing. Contributing to the accident was the pilot did not use a checklist.
Jul 14, 2009 Whitehorse, CA N9138E Fatal (1) substantial Investigation pending
Aug 14, 2007 Arctic Village, AK N5660F Fatal (2) destroyed The pilot's failure to maintain sufficient airspeed to avoid a stall/spin while maneuvering.
Aug 15, 2004 Seward, AK N6194M Fatal (2) substantial

The failure of an aviation mechanic/inspector to adequately inspect the pilot/owner's work during the engine overhaul, which resulted in a loss of engine oil and engine power during cruise flight, and a subsequent ditching. Factors associated with the accident were the inadequate installation of the idler gear shaft setscrew, and the lack of a safety wire on the three-quarter inch hex head (oil) plug in front of the idler gear shaft. Additional factors associated with the accident were the lack of suitable terrain for a forced landing, and ocean water.

Jun 22, 2003 Rockvale, TN N555MR Fatal (3) substantial Loss of engine power for undetermined reasons.
Jul 15, 2001 Bettles, AK N9237E Fatal (4) substantial The pilot's inadvertent stall of the airplane during an unknown phase of flight.
Apr 16, 2000 BABB, MT N284X Fatal (1) destroyed Terrain clearance was not maintained. High mountains, mountain obscuration, and the pilot's inadequate preflight planning/preparation were factors.
Jun 22, 1999 LAKE ST. JOHN, CA N62041 Fatal (2) substantial Investigation pending
Jun 06, 1998 KETTLE FALLS, WA N16CB Fatal (2) destroyed The pilot's failure to maintain terrain clearance. A factor was an attempt to perform aerobatics at low altitude.

The pilot's failure to maintain aircraft control while flying in instrument meteorological conditions. Factors were the weather, and the failure of the airplane's vacuum pump which rendered the attitude indicator and the directional gyro inoperative.

Oct 23, 1995 Alta, WY N71AR Fatal (1) destroyed the corrosion, consequent fatigue cracking and ultimate separation of the right wing forward lift strut.
Apr 02, 1995 RUSH CITY, MN N56349 Fatal (1) destroyed THE PILOT'S POOR JUDGMENT IN ATTEMPTING TO LAND IN THE EXISTING WIND CONDITIONS. HIS LACK OF EXPERIENCE WAS A FACTOR.

the pilot's entry into a box canyon at an altitude insufficient to maintain clearance from the surrounding terrain and obstacles. Factors in the accident were the high density altitude condition and the steep, rapidly rising nature of the mountainous terrain.

May 23, 1992 WETUMPKA, AL N62047 Fatal (1) substantial THE PILOT'S INADEQUATE VISUAL LOOKOUT, AND HIS FAILURE TO MAINTAIN PROPER CLEARANCE FROM THE TRANSMISSION WIRES.
Oct 16, 1991 MORAN, WY N19AR Fatal (3) destroyed

The pilot's failure to maintain clearance and altitude from terrain while maneuvering, and his failure to adequately compensate for the wind conditions. Factors contributing to the accident are terrain-induced turbulence, unfavorable winds, gusty high winds, and low altitude flight.

Aug 17, 1991 RUBY, AK N9026E Fatal (3) destroyed A LOSS OF CONTROL INFLIGHT DUE TO IMPROPER AIRCRAFT HANDLING AND ALCOHOL IMPAIRMENT.
