The pilot’s exceedance of the airplane’s critical angle of attack while maneuvering for an emergency landing following a partial loss of engine power, which resulted in an aerodynamic stall/spin and impact with terrain. Contributing to the accident was the partial loss of engine power due to carburetor icing.

Aug 28, 2021 Troy, AL N32566 Incident substantial The pilot's failure to maintain directional control while landing which resulted in a collision with terrain.

The pilot’s failure to transfer fuel from the auxiliary fuel tanks to the main fuel tanks in a timely manner, which resulted in fuel starvation to the engine.

Sep 03, 2012 Manzanita, OR N353LS Incident substantial The pilot’s inadequate compensation for the crosswind during landing, which resulted in a loss of directional control.
Jul 05, 2011 New Carlisle, OH N800NH Incident substantial The pilot's failure to conduct a go-around once he recognized he was going to land long on the runway.
Oct 15, 2010 Almyra, AR N125HS Incident substantial The pilot’s improper touchdown point and delayed decision to execute a go-around, which resulted in a runway overrun and collision with terrain.
Oct 02, 2010 Magnolia, AR N152HS Incident substantial

The pilot's inadequate compensation for gusty crosswind conditions and failure to maintain proper runway alignment that resulted in dragging a wing and ground looping. Also causal was the pilot's delayed decision to abort the landing.

Apr 24, 2007 Sierra Blanca, TX N800CP Incident substantial The loss of control as result of the pilot's failure to compensate for the existing wind conditions. A contributing factor was the crosswind.
Feb 11, 2007 Alton, NH N10438 Incident substantial

The pilot's failure to maintain control of the airplane during the takeoff roll. Factors were the slick condition of the runway, and the lack of markings to denote the runway safety area.

Jul 23, 2002 Trenton, GA N10550 Incident substantial
The failure of the pilot to clear the engine during a prolonged approach at idle power resulting in the engine hesitating during application of power …

The failure of the pilot to clear the engine during a prolonged approach at idle power resulting in the engine hesitating during application of power to arrest the descent for landing touchdown resulting in a hard landing and separation of the left main landing gear wheel. Contributing to the accident was incomplete fusion of the weld joint between the left main landing gear axle and wheel spacer and the lower than normal idle setting of the engine.

Jan 07, 2001 Perry, FL N40030 Serious (1) substantial The loss of control on landing due to the student's improper recovery from a bounced landing, and the resulting nose over on the grass runway.
